Skilled Nursing & Assisted and independent living
We are currenlty servicing clients in a variety of senior communities including: Skilled Nursing, Assisted Living and Independent Living communities
Skilled Nursing
- PT, OT and ST disciplines will provide rehab to residents either in their private rooms or shared gymnasium
- Coordinate with RN team to ensure medical precautions, patient schedules, and family coordination
- Train staff on patient guarding, transfer safety, and injury prevention
Assisted & Independent Living
- PT, OT and ST to provide care in the patient’s room, common space or in community
- Establish goals and interventions to improve quality of life and injury prevention
- Coordinate with RN supervisors and family to ensure continuity of care
- Focus on safe community participation and maintaining independence
