Provide individualized therapy in patients’ homes, helping them work toward improved daily living skills, safety and functional independence. Share your preferred coverage territory and availability so we can match appropriate referrals without excessive travel.

Who we’re looking for
We value clinicians who are dependable, responsive, professional and dedicated to excellent patient care. Ideal applicants:
Hold an active Illinois OT license
Have home health experience or are comfortable working independently with appropriate clinical oversight
Communicate promptly about availability, scheduling and patient needs
Complete accurate documentation in a timely manner
Work effectively with patients, families and the broader care team
Schedule and coverage
Schedule: PRN, part-time or potentially full-time caseload, based on referral availability. Specific caseload size is not guaranteed and depends on referrals and territory.
